ABSENCE REQUEST FORM

REMINDERS:

This website conveniently provides a calender in which mostly all appointments and absences can be worked around. Please utilize it and check it regularly.

Band is considered a CO-CURRICULAR activity, meaning that all after-school/weekend rehearsals and performances are factored into your final grade. This is different than extra-curricular. Please prioritize your scholastics accordingly.

Rules for excused and unexcused absences

1) You must submit your request at least one (1) week before the potential absence.

2) Just because you submitted a request does not mean that your absence is excused. Mr. Cabrera must still approve of your request.

3) This form is meant to be a LAST RESORT. Please attempt to schedule all appointments or other needs on days that do not conflict with you commitment to the music program.
